When needing to use a room in the church, please fill out this request form and return to Kelly Sartorius no later than 14 days before the event. This is a PDF fillable form. Please save the document as "Facilities Request - Your Name" and email to kellysartorius@fpcnorthville.org. Thank You!
Off-Site Activity Participation Form
Children and youth participating in programs off-site that are sponsored by The Church will need to fill out this form. Examples include the Mission Trip or High School Retreat.
